l: low
m: medium
h: high
u: ultrahigh

These are given in the subroutine, nfqlty, according to a Japanese manual.

The number of k-points used for the integration depends also on the lattice, however you can check the output file, it tells how many were used, just play arround with the settings and note them down, or have a look in the source code.
